To apply you can call your local Social Security office; or call the national number at, 1-800-772-1213; or go on line to www.socialsecurity.gov.
When you call they will take your application by phone. They will send you some forms to fill out and they will later ask you to come by your local office to be interviewed. When you go in for the interview you should take the following with you:
- Your Social Security Card or number;
- Your birth certificate;
- Your children's birth certificates if you have children;
- Proof of U.S. Citizenship or lawful alien status, if applicable;
- Your spouse's birth certificate and Social Security number if you are married;
- Your marriage certificate, if applicable;
- Your military discharge paper if you in the service;
- Your most recent W-2 and tax return; and
- Information, which you may need to answer questions about your assets and income, if any
You should always apply for all benefits you may be entitled to. There are two types of programs for the disabled. Social Security Disability, SSD, is the federal disability insurance program you pay into every time you get a pay check. The other program is Supplemental Security Income, SSI. This program is based on financial need and for people who have not worked enough quarters to be insured, or their insured status expired in the remote past.
